Enaya and Daam Specialized Care Center
Enaya and Daam Specialized Care Center has implemented PCC practices that promote partnership and enhance communication process for the patients, such as the Collaborative Care Conference to facilitate communication exchange between staff and patients, the Arabic Learning Whiteboard for patients to learn new Arabic words each day, the Patient/Family Note to support open communication between patients and their family members, the Social Determinants of Health where selected patients are assessed by a social worker, and the Care Partner Program to increase the number of care partners. PCC initiatives were set in place to recognize what matters most to individuals. These things were identified as Patient and Family Concerns which are addressed by Nesma’ak Customer Services, the Patient and Family Preferences and Wishes, Meal Preferences of the patients, availability of consultation rooms and dental clinics, Spiritual Services for various religious groups, Staff Concerns, Staff Satisfaction Survey, Staff Health Wellness and Wellbeing Program, and a Physical and Emotional Health and Grief Support (THIQA and AMAN). Enaya also hosts outdoor activities that promote engagement among the patients, family members, visitors, and staff, such as outings, strolling at the park, barbecue parties, family gatherings at the garden, National Sports’ Day celebration, and indoor activities like Arts and Crafts and Music Therapy Sessions.
Communicable Disease Center
Communicable Disease Center’s dedication to delivering compassionate and comprehensive patient care is exemplified through its initiatives aimed at enhancing patient wellbeing, such as the Therapeutic Art sessions for inpatients, a Book Donation Drive supporting the PCC Library, and active participation in the National Flu Vaccination Campaign. These efforts reflect CDC’s dedication to providing holistic care beyond medical treatment.
Ambulatory Care Center
Ambulatory Care Center’s innovative programs are designed to enhance patient experience and strengthen community engagement. These initiatives promote meaningful interactions and active participation among patients, their families, and the broader community, contributing to the continuous improvement of healthcare services. The Minhati Program inspires youth to explore healthcare careers through workshops and interactive sessions in a realistic setting. Under the Minhati program, in partnership with HOPE Qatar, Minhati HOPE provides children with special needs the opportunity to learn about medical care and interact with medical practitioners. Ambulatory Care Center has also partnered with schools in developing the Students’ Health and Wellbeing Program, a student health and wellbeing hub that provides workshops, learning activities and videos covering topics such as healthy eating, physical activities, exam stress management, managing emotions, bullying, youth mental health, and personal hygiene with a focus on infection control practices. To further enhance care experience at the hospital, the Care and Help Patient Program was designed to offer wellness activities during the hospital stay and post-discharge, including the iCARE for cancer patients. In collaboration with CPESE, several initiatives were successfully implemented to improve overall healthcare experiences and ensure compassionate care delivery across all aspects of its operations, which include the PCC Ambassadors program, the PCC Library, and the Hello My Name is… campaign.
Home Health Care Services
Home Health Care Services understands that family and friends are a vital part of a patient’s healing process. Through the Care Partner Program, patients can assign a loved one to get actively involved throughout the care encounter of the patient, which may include advocacy needs, educational needs, emotional support needs, general care needs, health care needs, and safe use of medical equipment. The My Care Wishes form offers patients with a structured method to communicate their care preferences to their healthcare providers and loved ones, empowering them to advocate for their own healthcare journey. Educational materials, including leaflets and videos, were collaboratively developed with PFAC members and available on the HMC-HHCS website for convenient access by patients and their families. By establishing a decentralized Patient-Family Focus Groups, Home Health Care Services incorporates patient and family perspectives to drive significant enhancements on various aspects of healthcare delivery.
Private Nursing Services
Designed to keep Qatar's community updated on services offered by Private Nursing Services, the HMC-PNS website ensures easy access to care and minimizes the need for in-person visits. Case managers at HMC facilities can also request nursing services for patients using the PNS forms. Designed to improve communication between assigned nurses and families of dementia patients, the My Care Passport supports more individualized nursing care, enhancing care preferences and communication for people living with dementia. In collaboration with Hamad International Training Center, Private Nursing Services offer free Basic Life Support and First Aid Training for the Care Partners, with sessions conducted in English and Arabic languages. Through the Tracheostomy Management Competency Fair, the community nurses were equipped with the necessary knowledge, skills, and attitude to enhance the management of tracheostomy patients and handle emergency.
Women’s Wellness and Research Center
To support informed healthcare decisions, transparency of data is core to Women’s Wellness and Research Center, by introducing essential data presented in three categories for public access: (1) Clinical Quality Measures; (2) Efficiency Measures; (3) Service Excellence Measures. To enhance nursing care safety, the hospital launched several PCC best practices, including the Bedside Shift Reporting, a structured approach that mitigates communication pitfalls seen in traditional methods, promoting transparency and active patient involvement in care decisions; the Care Partner Program that enhances maternity care services by involving families in the patient’s care journey from admission onward; and the Ahlan Whiteboard that facilitates patient-staff communication, personalizing care by integrating personal preferences and lifestyle goals into the care plan. In advocating for women’s overall health and wellbeing, Women’s Wellness and Research Center has partnered with Qatar Cancer Society to enhance healthcare outcomes and provide comprehensive care tailored to cancer patients’ medical and emotional needs.
Al Wakra Hospital
Al Wakra Hospital held informative educational sessions titled, Shared Decision-Making: A Vital Component of Person-Centered Care, to emphasize the importance of involving patients in their healthcare decisions. The significance of Shared Decision-Making and Advance Care Planning were presented to all service leaders of the hospital, to emphasize their role in fostering and practicing person-centered care. Several quality improvement projects were developed, including the Medication Reconciliation which aims to increase medication reconciliation within 24 hours of admission; the Outpatient Department (OPD) Efficiency and MRI Wait Times which aims to reduce waiting period for OPD visits; the Real-Time Patient Feedback which ensures that any gaps identified through patient interviews are promptly addressed; a pilot Care Partner Program in Burn Unit, Peritoneal Dialysis Unit, and Pediatric Long-Term Unit which allows designated family members or friends to be involved in patient care; the Whiteboard which was co-designed with PFAC members to serve as a central communication hub and ensure effective communication of vital patient information during shift handovers; real-time Access to Personal Health Information to empower patients and caregivers to make informed decisions; assessments for Social Determinants of Health and health literacy was implemented across the hospital; the Visiting Hours Guidelines was revised to reflect best practices for patient safety and recovery; and the Audit Tool was developed to evaluate implemented practices and identify areas for improvement.
Heart Hospital
Heart Hospital actively partners with organizations such as the Ministry of Awqaf and Islamic Affairs (AWQAFM, also known as the Ministry of Endowments and Islamic Affairs), Center for Indian Community (CIC), and Ehsan (Center for Elderly Empowerment & Care) to raise awareness about cardiovascular disease and provide access to cardiology services. These collaborations extend to local businesses, offering discounts and complimentary services for wellness and beauty, further enriching patient care and community health initiatives. Various international, national, and religious occasions are also celebrated and participated by both staff and patients, including events like National Sports Day, Garangao, World Heart Day and Eid Al-Adha, promoting a sense of community, teamwork and a festive atmosphere within the hospital.
