The Certification for Excellence in Person-Centered Care by Planetree International represents the highest mark of achievement for creating an organizational culture that engages patients, families, staff, and the community to improve overall health and wellbeing. This evidence-based program provides a structured pathway and operational framework for establishing the systems and processes necessary to sustain culture change across organizations.
In 2020-2021, five HMC facilities received Planetree International’s Certification for Excellence in Person-Centered Care after designing care initiatives and benchmarks, cultivating a culture of continuous improvement, and consistently orienting care around patients, families, and communities.
The certification criteria address components of a person-centered healthcare experience, including the quality of patient-provider interactions, access to information, family involvement, and the physical environment of care. The criteria also focus on how the organization supports staff, opportunities for staff, patients, and families to have a voice in the way care is delivered, and the ways that the organization is reaching beyond its walls to care for its community.
