Purpose Becoming an Age-Friendly Health System entails reliably providing a set of evidence-based, geriatric best-practice interventions across four core elements, known as the “4Ms,” to all older adults in your system. The 4Ms are the essential elements of high-quality care for older adults and, when implemented together, represent a broad shift by health systems to focus on the needs of older adults.

The Format:

A “recipe” for reliable integration of the 4Ms into your standard care has steps and ingredients, just like a recipe to make a salad, main dish, or dessert. These steps include:

  1. Get ready to assess and act on the 4Ms
  2. Define what it means to provide care consistent with the 4Ms
  3. Design/adapt your workflow to deliver care consistent with the 4Ms, including how you will document the 4Ms
  4. Provide care consistent with the 4Ms
  5. Study your performance – how reliable is your care? What impact does your care have?
  6. Improve and sustain care consistent with the 4Ms
