Keynote Lecture:
Root Cause Analysis (RCA) for Improvement of Healthcare System and Patient Safety
This plenary address is intended for healthcare professionals leading the analysis of patient harm events, those executives and leaders directing and managing patient safety, and those advocating for evaluation of patient harm using best practices.
Learning Objectives:
- Create an understanding of the need, tools, applications, structure, and methodology of RCA for analysis and action planning
- Understand the roles necessary to support successful RCAs
- Discuss countermeasures to prevent RCA failures, and strategies to prevent repeat harm events
Note: This is a prelude to RCA workshops on Days 2 and 3
