As a patient receiving this customized exercise program, I would like to declare the following :-

• 1. I will not share my username & password to others.
• 2. I will not disseminate the content with others as it is designed for my condition & may harm other patient’s condition as they were not assessed by Physiotherapist.

بصفتي مريض يتلقى برنامج التمرين هذا المخصص لي ، أود أن أقر بالآتي:-

• 1. لن أشارك اسم المستخدم وكلمة المرور الخاصين بي مع أي شخص آخر.
• 2. لن أنشر أي من محتوى التمارين مع أي شخص آخر لأنها تم تحديدها مسبقا بناء على حالتي الصحية وقد تضر في حالة استخدمها أي مريض لم يتم فحصه من أخصائي علاج طبيعي.
