Dr. Hanan Saleh Alyazeedi A Alyafei earned her Medical degree in 2000 and Doctorate in 2002 from Arabian Gulf University, Bahrain and served residencies at Hamad Medical Corporation in 2003. She has completed her Arab Board Training Program in Family Medicine in 2008 and Certificate in Quality Management from Medical Education Hamad Medical Corporation in 2018.
She was appointed as the Deputy Medical Director of Hamad Medical Corporation, Home Health Care Service in September 2016. As the Deputy medical director; she is responsible for overall management of HHCS for ensuring and maintaining the quality and efficiency of clinical services and professional performance of physicians. She worked as Consultant in Primary Health Corporation, Gharaffa Al Rayyan from 2008 – 2016 before being appointed as the Deputy Medical Director. She joined in Hamad Medical Corporation, Home Healthcare Service on 2016 as Consultant.
She has more than 17 years of medical practice experience in Primary Health Corporation and Hamad Medical Corporation in various levels as resident, specialist and consultant. In PHCC, as a specialist and later as Consultant she managed patient conditions through multidisciplinary approach according to patient’s need and ability and lead the MDT when required. She was also the focal point for Antenatal clinic and mental health clinic at her health center. Since she joined Home Healthcare Services she has been actively involved in the management of diverse chronic general medical and geriatric problems in home care settings. She was also responsible for regular and emergency home visits for treating patients at their home.
She was elected Clinical Guide and Project lead for SEPSIS and Community Antimicrobial Stewardship Project in HHCS, HMC. She is a Leader and member of IA Working Group. She is actively involved and member of National Patient Safety Collaborative and Healthy Ageing.
Member of Patient Experience Forum and Clinical Champion for the Planetree Person Centered Care Program in HHCS. She was also speaker for Middle East Forum 2019.
Furthermore, she led and acted as the clinical advisor for various quality improvement projects in Home Healthcare Services.
She was elected as speaker for PEF Forum 2019 on review of her great achievements.
