Technology alone will not fix healthcare. This session explores how the future of healthcare must be intentionally designed around people, not just digital solutions. In January 2024, the first End-of-Life (EOL) Home Care Program was launched under Hamad Medical Corporation’s Home Healthcare Services (HHCS). This pioneering initiative addresses a critical gap in palliative care for older adults, enabling patients aged 60 and above with life-limiting conditions to receive compassionate, person-centered care in the comfort of their own homes. The program provides 24/7 interdisciplinary support, focusing on pain and symptom management, emotional and spiritual needs, and empowering families through education and engagement.

This session showcases the PCVC capacity-building program developed by the Council of Health Insurance (KSA) to advance value-driven, person-centered healthcare. Co-designed with Planetree International and tailored to the local context, the program has two tracks: staff capability building and organizational capacity development.
The staff track features a three-tier curriculum (Foundation → Proficient → Expert) aligned with an organizational PCC framework, structured around three domains—Structure, Engagement, and Culture—supported by 10 enablers and 80 standards. Over 90 healthcare professionals completed training, with Expert-level participants conducting guided maturity assessments across 29 hospitals in the private, government, and semi-government sectors.
Attendees will leave with a proven methodology for assessing and advancing person-centered care maturity, practical lessons learned, and strategies to scale this program further.

Transitions of care are challenging for both patients and families, especially when recovery continues beyond the hospital. To bridge gaps in care continuity, the Care Partner Program at HDU-C, Heart Hospital, was created to fill gaps in continuity of care by involving caregivers as active partners. The program equips families with knowledge, confidence, and clear roles to work effectively with healthcare teams. By using the IHI Model for Improvement, the program introduced structured protocols, orientation, and educational support to ensure smooth transitions from hospital to home.

This session highlights the journey of implementing the Care Partner Program at the Women’s Wellness and Research Centre (WWRC), Hamad Medical Corporation, Qatar. Traditionally, male family members were not permitted at WWRC—whether in labor and delivery rooms or even while accompanying their wives for antenatal appointments—often leading to emotional isolation for mothers. Through close collaboration with patients, families, and staff, WWRC introduced a structured Care Partner Policy that allowed husbands and family members to actively participate in childbirth, extending from postnatal units to labor rooms and even operating theatres during cesarean births. The presentation will share the program’s implementation strategies, staff orientation and training, real-life patient stories, challenges encountered, and measurable outcomes, as well as its alignment with global standards such as WHO, Planetree, and IPFCC. At Al Maha Pediatric Specialized Care Center (AMPSCC), a 60-bed facility providing comprehensive care for children with complex disabilities, a new infotainment system was launched in March 2024 to address digital accessibility gaps. Previously, many patients with severe mobility limitations were unable to independently use conventional devices, leaving them reliant on caregivers for even basic entertainment or educational access.
By integrating eye-gaze systems, children can now navigate digital platforms, access educational content, play games, and communicate with ease—using only their eyes. Participants will learn about the challenges of traditional handover practices, the benefits of patient-inclusive approaches, and the steps taken to standardize bedside handover using checklists, training, and feedback mechanisms. The session will include a review of process maps, driver diagrams, and PDSA (Plan-Do-Study-Act) cycles used to drive change, as well as results and lessons learned from the pilot implementation.
