In order for Hamad’s Health Information Management team to release the details from the appropriate medical records for any patient, you need to complete the request form.
There are several steps involved in requesting a report – please prepare for them in advance to speed up the process:
- Have all the relevant details ready (including Qatar ID, Health Card number and contact details of all parties concerned)
- Complete the online request form and supply the requested details
- Pay online using your credit or debit card
- Wait for SMS notification to confirm receipt of your request.
How long does it take to receive your request?
• A medical report takes up to 14 days to process.
• Copy of laboratory/diagnostic report takes three working days.
