When we feel your baby is ready to go home, a physician, your primary nurse and your case manager will discuss our discharge plans with you and your family and this will be entered into your electronic patient file.
We want to ensure your baby’s recovering well, so you may be scheduled at one of the specialized clinics below for a follow up appointment based on your baby’s condition and needs.
Once your appointment dates are available, our Nesm’ak will call and inform you of the date, venue and time of your appointment. For your added convince, you will also receive a text message closer to the date.
