Referral form issued and signed by Pediatric consultants/ physicians from HMC facilities or Primary Health Centers

Information about discharge process from your services:  

Discharge process is initiated using the following criteria:
  • Predetermined goals have been achieved partially or fully
  • Family or significant caregivers have been educated/trained in the set treatment programs
  • Poor compliance by the family on carrying over home program and /or attending educational training sessions
  • The family / care giver decides to seek another intervention locally or overseas
  • In case patients request leave during the admission period, discharge is based on reasons for leave, the age of the child and potentials for improvement
  • If the child has been absent without genuine reason for up to two weeks without informing the department.
Discharge process varies across services and programs.
Pre-Integration Program
Discharge process is initiated following 4-6 months episode of admission. Recommendations are individualized as determined by the interdisciplinary team:
Children over 6 years- are recommended for Final Discharge from the program with Home Program. Other recommendations as applicable: Referral for School Program Evaluation, Integration into regular school or Special Needs Center, Referral to other rehabilitation services
 
Children under 6 years -are either recommended for: reenrollment into the program (physician’s referral required), referral to regular pre- school with Home Program, Referral to other rehabilitation services as needed
