Home Health Care Service


HMC’s Home Healthcare Service (HHCS) delivers hospital-level care directly to patients’ homes, supporting individuals of all ages who are temporarily or permanently housebound. Led by multidisciplinary teams of doctors, nurses, clinical pharmacists, allied health professionals and care assistants, the service offers 24/7 ‘Hospital at Home’ care within the comfort of the patient’s home, delivered by experienced and specialised physicians. The model not only ensures high-quality clinical care but also enhances emotional wellbeing and enables families to play an active role in the care journey.

Key services


Management of patients with complex cases

  • HHCS is uniquely positioned to manage complex and specialized clinical cases outside the hospital. in 2025, around 80 patients dependent on mechanical ventilators are being cared for at home.
  • The service also provides critical treatments such as tracheostomy care, enzyme replacement therapy, long-term oxygen therapy, advanced wound management, and Point of Care Testing.

The Early Supported Discharge Program (ESD)

  • The ESD program enables eligible patients to continue receiving clinical care at home following discharge from hospital.
  • ESD ensures continuity of care beyond the hospital setting while supporting patient recovery at home under expert supervision.

End-of-Life (Palliative) Care Program

  • Launched in 2024, HHCS’s End-of-Life (Palliative) Care Program provides compassionate, holistic care for patients with life-limiting illnesses - delivered within the comfort of their own homes.
  • This program focuses on symptom management, emotional and spiritual support, and ensuring patients can spend meaningful time with loved ones during their final days.​

  • Available to registered HHCS patients, the HHCS Call Center operates 24/7, responding to over 52,000 calls in 2024 alone.
  • It plays a crucial role in providing real-time guidance and reassurance to registered patients and their families, helping to avoid unnecessary hospital visits and ensuring continuity of care.
